To all of my wonderful patients:

It is with a heavy heart that I am announcing my departure from Central Family Practice in July. I have an opportunity with Resilient Health to further advance my training in functional medicine and participate in research that I hope will benefit the lives of many people. This was a difficult decision, but one that I feel is best for my intellectual interests and growth as a physician.

The relationships I have with my patients are what I value most about being a family physician. I am deeply grateful that you have allowed me to care for you and be a part of your lives. I will miss seeing each and every one of you and I will miss being a part of the wonderful family that is Central Family Practice. But I am leaving you in the very capable hands of my colleagues at CFP. Many of you have met our newest addition, Dr. Jennifer Ziedonis. She is a board certified internal medicine physician who is also a certified practitioner by the Institute for Functional Medicine. Dr. Christine Chen is a board certified family physician and osteopath who can treat a wide range of conditions. And of course, Dr. Elliot Trester and Eva Montes, PA-C remain available for your family practice needs. As Dr. Ziedonis and Dr. Chen are our newer physicians, they have more availability in their schedules. I will be working closely with them to make this transition as smooth as possible and I am certain you will continue to receive excellent care from everyone at Central Family. In order to receive medication refills from CFP, please have your follow-up appointment when it is due, which is at least every six months (you may need to be seen more frequently for certain medications, like controlled prescriptions).

My last day of patient appointments will be Thursday, July 11, 2024. Any appointments prior to that date will proceed as scheduled. For those of you with appointments scheduled with me after July 11, the CFP staff will be contacting you about transitioning to another provider. I regret that I will not be able to see each of you before I go but unfortunately, I do not have space in my schedule to accommodate that. And for those of you I have seen recently, I apologize I could not notify you in person until our staff was aware of the change. Your medical records will remain under the custodianship of Central Family Practice. You have the right to choose your healthcare provider and should you choose to have your records transferred to another practice, you may complete a medical records request form which is available on our website. Please know that it has been an honor to care for you at Central Family Practice for the last ten years.

Additional information: 
  • If you have been seen for your regularly scheduled appointments in the last 6 months, CFP will refill your medication, but you will need to establish with the practitioner that Dr. Pugh has chosen for you before your next refill is due.  Patients who are on controlled medications (i.e., stimulants for ADHD) will need to be seen every 3 months with Dr. Chen or Dr. Trester, so please plan accordingly.
  • After her departure, your lab results will come to the provider Dr. Pugh has chosen for you. They will review the results with you. If there are any unanswered questions, they have the ability to contact Dr. Pugh. Lab results that finalize prior to July 12th will be addressed by Dr. Pugh.
  • If you have an appointment scheduled after July 11th, the front office will be contacting you in the coming weeks to reschedule you with the provider Dr. Pugh has chosen for you. Please be patient with this process as it will take some time for them to contact everyone.
  • If you do not already have an appointment scheduled, Dr. Pugh’s schedule is currently full and she will not be able to accommodate any additional appointments. She requests that you go ahead and schedule with Dr. Ziedonis or Dr. Chen to establish care with them.
